Title: SMS posts forms for Membership, Minor Consent, Season Passes Post by dscEvents on 01/13/10 at 10:48am We have now posted printable PDF versions of the SMS Membership form, the Minor Consent Form (renewable each year), Season Passes for admission and reserved seating, and a printable W-9 Tax form for all of our drivers. Remember, we need to have ALL Minor Consent forms and W-9's re-signed for the 2010 season. They do not roll over from 2009! All forms are printable. http://www.racesantamariaspeedway.com/ Just go to the TEAM INFO drop down and select FORMS, then select the form you want to print.
